| Advanced Standing Form | Usage: Allows student to sign up for Advanced Standing Exams. This does not apply to Research or Design courses such as CS 4911. |
| Computer Science Minor Form | Usage: Used to petition for a Minor in CS upon graduation. Due: Should be submitted for approval by a CS advisor with the petition for a degree. |
| Petition to Faculty Form | Usage: Must be completed by a student when asking for an exception to an Institute rule or policy. Due: Should be submitted for approval by a CS advisor at least one week prior to an Institute Undergraduate Curriculum Committee meeting. |
| Undergraduate Senior Request to Register for Graduate Course | Usage: Used by undergraduate senior students who are attempting to register for graduate courses. Due: Prior to the end of Phase II Registration. |
| Undergraduate Research Permit Form | Usage: Allows students to request permission to register for Undergraduate Research courses. Due: If the student is registering for a CS 4980 section, this permit must be submitted to Student Services by the end of finals week during the semester prior to the semester which the student plans to enroll in CS 4980. | Graduate Student Exit Survey |
Usage: To be completed by graduate students in the MS CS, MS InfoSec and CS PhD programs in conjunction with the petition to graduate. Due: Prior to the deadline for graduation petitions. |
| Request for Transfer of Graduate Credit for CS Courses | Usage: This form is used to request graduate level credit for courses taken at other institutions. Credit can be used to reduce the course requirements or to meet curriculum requirements, subject to the limits of the Institute and specific program. Due: Typically no later than the end of the first semester of enrollment. |
| Undergraduate Request to Register for Graduate Course | Usage: This form is used by undergraduate student who are attempting to register for graduate sections. Due: Prior to the end of Phase I Registration. |
| Student Request for Travel Funds | Usage: This form is used to request reimbursement of travel expenses associated with attending a conference, workshop or other sponsored activity. Submission of this form does not ensure a student will receive funding. Limited funds are available for such purposes and students should request funding as soon as the need is known (acceptance of a paper, etc.).
